CONTROLLER

This form has been prepared by Şoteks
Label Technologies Ind. Trd. Inc. based on the 13th article of the Personal Data Protection Law No. 6698, and includes the right of the persons whose personal data are processed(“Relevant Person””) in
Article 13 of the Law to apply to the data controller. Accordingly, our Company has arranged the application form as follows. The requests included in the application made to our company will be concluded free of charge as soon as possible and within thirty days at the latest, depending on the nature of the request. However, if the transaction requires an additional cost, the fee in the tariff determined by the Personal Data Protection Board (“Board”) may be charged. 

Our company may accept the request of the Relevant Person or reject it by explaining the reason. It notifies the Relevant Person of the application in writing or electronically. In the event that the request in the application is accepted, necessary actions are taken by our Company. In case the application is caused by the fault of our Company, the fee collected is returned to the Relevant Person. The Relevant Person must submit their requests regarding the implementation of the Law to our Company in writing or by other methods to be determined by the Board. In this context, applications to be made to our Company in “written” form; by printing out hereby this Application Form;

• By Relevant Person, the form available at www.soteksetiket.com must be sent with a secure electronic signature to [email protected] via registered e-mail (REM). In addition, other application methods that can be determined by the Board will be announced to you by our Company.
